- Head on to our Facebook Page at https://www.facebook.com/CARAPhil and click on the “Online Reservation: Special Screening of MAX” tab. If you’re using your mobile phones, head directly to http://www.jotform.me/alpalobaloba/onlineReserveMAX
- Fill the necessary details and submit.
2. At the CARA Clinic
- CALL our clinic at 5323340 from 1:00PM to 5:00PM (look for Stephanie).
- Leave your name, phone number and e-mail address.
- Inform us of the number of seats you will reserve and the corresponding seat numbers.
Please note that reservation is valid for only 3 days. Failure to pay within 3 days will cause forfeiture of your reservation.

1. CARA Clinic
Look for Stephanie or Paula. Inform them of your seat numbers upon payment.
2. BPI
Cash deposit to CARA Welfare Philippines BPI Current Account # 3191-0467-05. E-mail the deposit slip and include your seat numbers. An e-ticket will be provided upon confirmation of your payment. Kindly present the e-ticket on the screening day to get your actual ticket and your freebies!
